A “Never Event” or a “sentinel event” is identified as something that should never happen in the exercise of reasonable care, such as operating on the wrong limb, a retained foreign body, and Stage IV pressure ulcers. “Never Events” are defined by Medicare, meaning that they will no longer pay for treatment related to these events because they are assumed to be caused by poor care on the part of the healthcare provider. Hospitals are required to report these sentinel events to the Joint Commission, who accredits hospitals.
